Allied health care in parkinson's disease: referral, consultation, and professional expertise
There is evidence for the efficacy of allied health care in Parkinson's disease (PD). However, barriers exist that hamper implementation of evidence into daily practice. We conducted a survey to investigate: (1) to what extent PD patients currently utilize allied health care for relevant problems in the core areas of allied health care and (2) the level of PD-specific expertise among allied health professionals. Questionnaires were sent to 260 patients and 297 allied health professionals. Referral rates were 63% for physical therapy, 9% for occupational therapy, and 14% for speech therapy. PD patients with problems that can potentially be alleviated by input from allied health professionals are often not being referred. Furthermore, most patients were treated by allied health professionals... ...»

Physical therapist management of acute and chronic low back pain using the world health organization's international classification of functioning, disability and health.
DISCUSSION:/b> The WHO-ICF model appears to provide an effective framework for physical therapists to better understand each person's experience with his or her disablement and assists in prioritizing treatment selection. The explicit acknowledgment of personal and environmental factors aids in addressing potential barriers. The WHO-ICF model integrates well with other models of practice such as Sackett's principles of evidence-based practice, the rehabilitation cycle, and Edwards and colleagues' clinical reasoning model. Future research should examine outcomes associated with the use of the WHO-ICF model using adequately designed clinical trials. Advancements in contemporary physical therapy research: use of mixed methods designs.
Advancements in Contemporary Physical Therapy Research: Use of Mixed Methods Designs. Phys Ther. 2008 Nov 13; Authors: Rauscher L, Greenfield BH The purpose of this article is to advocate for the use of mixed methods designs in contemporary physical therapist research. Mixed methods research is a design for collecting, analyzing, and mixing both quantitative and qualitative data in a single study or series of studies to both explain and explore specific research problems, thereby enriching the breadth and depth of understanding phenomena. These designs are particularly well suited for physical therapist researchers to reveal the complexity of disablement, rehabilitation, and recovery processes. Although contextual factors influence a person's health condition and recovery, they... ...»

Regulating intensity using perceived exertion: effect of exercise duration
Abstract  To examine whether the validity of perception-based intensity regulation would be affected by exercise duration, 20 subjects were recruited to complete a maximal exercise test (GXT) and four submaximal trials of varying duration and intensity using a cyle ergometer. During GXT, ratings of perceived exertion (RPE), oxygen uptake (VO2), heart rate (HR), and power output (PO) equivalent to 50 and 75% VO2peak were determined. During each trial, subjects were to produce and maintain a workload using RPE estimated at 50 or 75% VO2peak for 20 or 40 min, and VO2, HR, and PO were measured throughout the exercise. No differences in average VO2 were found between the estimation and production trial of either duration. However, average HR and PO were lower (P < ... ...»

Does continuing education improve physical therapists' effectiveness in treating neck pain? a randomized clinical trial.
CONCLUSION:/b> The results of this study demonstrated that ongoing education for the management of neck pain was beneficial in reducing disability for patients with neck pain while reducing the number of physical therapy visits. However, changes in pain did not differ for patients treated by the 2 groups of therapists. Although it appears that a typical CE course does not improve the overall outcomes for patients treated by therapists attending that course, more research is needed to evaluate other educational strategies to determine the most clinically effective and cost-effective interventions. Cauda equina syndrome in a pregnant woman referred to physical therapy for low back pain.
Authors: O'Laughlin SJ, Kokosinski E The patient was a 38-year-old female at 34 weeks' gestation, with a 3-week history of low back pain of insidious onset. Her physician referred her to physical therapy on the assumption that her pain was typical of normal pregnancy. However, the patient's significantly worsening condition prompted the physical therapist to contact the physician who ordered lumbar magnetic resonance imaging. MRI results led to a diagnosis of cauda equina syndrome, secondary to central spinal canal stenosis from a posterocentral disc extrusion at L4-5. The patient underwent an immediate surgical discectomy at L4-5.J Orthop Sports Phys Ther. 2008; 38(11):721. doi:10.2519/jospt.2008.0411. Patellofemoral joint force and stress between a short- and long-step forward lunge.
CONCLUSIONS: When the goal is to minimize patellofemoral joint force and stress during the forward lunge performed between 0 degrees to 90 degrees knee angles, it may be prudent to perform the lunge with a long step compared to a short step and without a stride compared to with a stride, because patellofemoral joint force and stress magnitudes were greater with a short step compared to a long step at higher knee flexion angles and were greater with a stride compared to without a stride at lower knee flexion angles.J Orthop Sports Phys Ther. 2008; 38(11):681-690, Epub 24 October 2008. doi:10.2519/jospt.2008.2694. Dynamic trunk stabilization: a conceptual back injury prevention program for volleyball athletes.
Authors: Smith CE, Nyland JA, Caudill P, Brosky JA, Caborn DN SYNOPSIS: The sport of volleyball creates considerable dynamic trunk stability demands. Back injury occurs all too frequently in volleyball, particularly among female athletes. The purpose of this clinical commentary is to review functional anatomy, muscle coactivation strategies, assessment of trunk muscle performance, and the characteristics of effective exercises for the trunk or core. From this information, a conceptual progressive 3-phase volleyball-specific training program is presented to improve dynamic trunk stability and to potentially reduce the incidence of back injury among volleyball athletes. Phase 1 addresses low-velocity motor control, kinesthetic awareness, and endurance, with the clinician providing cues t... ...»

Predictors of frontal plane knee excursion during a drop land in young female soccer players.
CONCLUSIONS: Frontal plane knee excursion during a drop land task was partially attributed to available range of motion at the hip and ankle. These results suggest that range of motion of the joints proximal and distal to the knee should be considered when evaluating individuals who present with excessive frontal plane knee excursion during this task. Given that the relationship between range of motion and frontal plane knee excursion was small, other factors, including learned motor patterns, should be considered.J Orthop Sports Phys Ther. 2008; 38(11):661-667; Epub 22 August 2008. doi:10.2519/jospt.2008.2695.
